The vast majority of root canals are carried out for extensive cavities, which reach the nerve and cause severe pain. On other occasions, they are performed in teeth with chronic pulp involvement, that is, teeth in which the nerve has already been lost due to an infection (necrosis) but does not hurt. In these cases, sometimes a fistula occurs, that is, a small lump that occasionally protrudes about 10 mm. above the crown of the affected tooth.
